Competing risks तब होते हैं जब कई पारस्परिक रूप से अनन्य घटनाएं पहले घट सकती हैं और एक घटना दूसरी को देखने से रोक देती है। किसी loan में default, prepayment, maturity या sale पहले हो सकते हैं; prepayment उस loan को आगे के default risk set से हटा देता है। Cause-specific hazard, अभी किसी भी event से मुक्त units में किसी एक cause की instantaneous rate बताता है, जबकि cumulative incidence यह वास्तविक probability देता है कि किसी horizon तक कोई cause पहले घटेगा। Competing events को सामान्य independent censoring मानने पर one-minus-Kaplan–Meier cause probability को अधिक दिखाता है।

पहली घटना आगे का risk set बदल देती है

जब prepayment या maturity पहले हो सकती है, तब default तक का समय अकेले observe नहीं होता। इनमें से कोई घटना होते ही मूल contract के तहत बाद का default संभव नहीं रहता।

ये events compete करते हैं क्योंकि first-event question के लिए उनका क्रम पारस्परिक रूप से अनन्य है। Event taxonomy को economic process के अनुसार होना चाहिए।

सभी exits को मिलाने पर किसी भी exit तक का समय estimate होता है। उन्हें अलग करने पर पूछा जाता है कि कौन-सा exit पहले होता है, जबकि competitors के बिना hypothetical world एक अधिक मजबूत और कम observable question पूछता है।

Ordinary censoring गलत probability देता है

Kaplan–Meier censored units को बचे हुए risk set के representative मानता है। Competing event इसके बजाय बताता है कि target event अब पहले नहीं हो सकता।

Default estimate करते समय prepaid loans को censor करना मानो उन्हें continuing loans जैसी ही future default opportunity मिली होती।

इसलिए वह Kaplan–Meier curve से one minus करने पर competitors को हटाने वाली net quantity estimate होती है और सामान्यतः real-world default cumulative incidence से अधिक होती है।

Cause-specific hazard वर्तमान event flow बताता है

Cause k के लिए cause-specific hazard उन units में cause k की instantaneous rate है जिनमें अभी कोई event नहीं हुआ।

Cox model इस hazard के साथ covariate associations estimate करने के लिए दूसरे causes को censor कर सकता है। यहां censoring risk-set device है, actual cause probability का estimate नहीं।

Cause-specific hazard ratios mechanism और etiologic questions के लिए उपयोगी हैं, लेकिन सभी causes के hazards के बिना वे cumulative incidence में सीधे map नहीं होते।

Cumulative incidence absolute probability का उत्तर देता है

Cause k का cumulative incidence function वह probability है कि time t तक cause k पहले घटेगा।

यह हर समय सभी causes से survive करने की probability के साथ वर्तमान cause-specific hazard को जोड़ता है। इसलिए competing hazards target probability को आकार देते हैं।

सभी cause-specific cumulative incidences किसी भी event की probability तक जुड़ती हैं और event-free survival को शेष भाग के रूप में छोड़ती हैं।

Fine–Gray subdistribution hazard को model करता है

Fine–Gray regression covariates को ऐसे subdistribution hazard से जोड़ता है जिसे cumulative incidence को सीधे model करने के लिए बनाया गया है।

इसके risk set में prior competing event वाले लोग weighting conventions के जरिए बने रहते हैं। इसलिए resulting subdistribution hazard ratio ordinary current-risk rate ratio नहीं है।

जब covariate association के लिए cumulative incidence target हो, तब इसका उपयोग करें। इसके coefficient को direct probability multiplier में न बदलें।

Covariates competing pathways से काम कर सकते हैं

किसी borrower feature से prepayment और default दोनों hazards बढ़ सकते हैं। तेज prepayment high-risk loans को default से पहले हटा सकता है और observed default incidence घटा सकता है।

इसलिए कोई covariate cause-specific default hazard बढ़ा सकता है, फिर भी default cumulative incidence के साथ उसका association कमजोर या विपरीत हो सकता है।

यह विरोधाभास नहीं है। दोनों estimands current event flow और realized first-event probability के अलग questions का उत्तर देते हैं।

Financial applications को complete event labels चाहिए

Credit studies में default, prepayment, maturity, restructuring और loan sale अलग किए जा सकते हैं। Fund studies में liquidation, merger और database disappearance अलग किए जा सकते हैं।

Event misclassification observations को causes के बीच ले जाती है और हर risk set बदल देती है। Ambiguous exit codes को चुपचाप censoring नहीं बनाना चाहिए।

Identification के लिए independent loss to follow-up अभी भी जरूरी है। Calendar cutoff, distress से correlated unclassified disappearance से अलग होता है।

Mechanisms और probabilities दोनों report करें

First-event question, event hierarchy, horizon, time origin, delayed entry, censoring और ties या ambiguous outcomes के समाधान को define करें।

Event-free survival और हर cumulative incidence plot करें ताकि probabilities reconcile हों। उपयोगी horizons पर uncertainty के साथ absolute incidence report करें।

Event dynamics के लिए cause-specific models और तभी Fine–Gray उपयोग करें जब उसका cumulative-incidence estimand decision से मेल खाए; हर ratio को साफ label करें।

आम सवाल

Default analysis में prepayments को censor क्यों न करें?

Cause-specific hazard estimate करने के लिए यह valid है, लेकिन one minus Kaplan–Meier से real-world default probability estimate करने के लिए नहीं।

क्या cumulative incidence समय के साथ घट सकती है?

नहीं। यह horizon तक cause के पहले घटने की accumulated probability है, इसलिए nondecreasing होती है।

Cause-specific Cox या Fine–Gray में कौन बेहतर है?

कोई भी सार्वभौमिक रूप से बेहतर नहीं है। Cause-specific models event dynamics address करते हैं; Fine–Gray covariate association को cumulative incidence से जोड़ता है।

क्या competing-risk probabilities का योग one से ऊपर जा सकता है?
